Verification Forms

All Verification Forms can be found via the FA Verification Portal. If your Free Application for Federal student Aid (FAFSA) has been selected for review in a process called verification, you will receive a To-Do List item directing you to create your FA Verification Portal account. You will be required to complete the online forms, e-sign, and upload all requested documentation via your portal. For more information, please visit the verification webpage.
