For Altamonte and Heathrow Campus Key and Prox Card Requests, complete and submit the Online Request.

Sanford/Lake Mary, Oviedo and Geneva Campuses

For Sanford/Lake Mary, Oviedo and Geneva Campuses, complete and submit the following appropriate form(s) to security@seminolestate.edu.  The below forms are in Adobe Acrobat format and are available to download and print.

• Key Request Authorization
• Prox Card Request Authorization
